Features of the OS 4

  • The unique pump innovation from Oertli. The SPEEP pump uses the same principle as a peristaltic pump to control the flow. With SPEEP the vacuum can also be controlled using the foot pedal. This enables precise control of the holding force generated right at the instrument opening.

    How does the SPEEP pump work?

    SPEEP combines the advantages of a flow-controlled peristaltic pump with the responsiveness of a vacuum-controlled venturi pump.

    The SPEEP pump allows both the flow and vacuum to be controlled independently of each other. The foot pedal not only allows aspiration and release but also gives the surgeon complete control when holding and manipulating fragments and tissue. SPEEP continues to control the vacuum even under occlusion.

    What are the benefits of the SPEEP pump?

    With challenging cases such as floppy iris syndrome or zonular weakness, precise control of the fluidics is essential. Thanks to the independent settings for flow and vacuum, SPEEP generates dosable holdability at the instrument opening — giving surgeons complete control.

  • Thanks to the use of the fluidics technology and the precise flow control, easyPhaco enables immediate aspiration of the lens material for perfect followability. The unique design of the tips ensures unmatched chamber stability while simultaneously providing very high holdability. The fragments are aspirated efficiently and without repulsion. The focused axial delivery of the ultrasound energy guarantees targeted emulsification directly into the lens fragment. And, because the Oertli easyPhaco handpiece is equipped with six Piezo crystals, the transmission of power to the tip happens in a direct and gentle way with less heat development.

  • For surgical treatment of glaucoma, the OS 4 generates appreciable added value in microinvasive glaucoma surgery (MIGS). High-Frequency Deep Sclerotomy (HFDS) has a low rate of complications and delivers promising long-term results even with a shorter procedure. HFDS provides direct access from the anterior chamber to the Schlemm’s canal and further into the sclera. The outflow resistance of the trabecular meshwork is thus significantly reduced. The HFDS glaucoma tip is inserted through a 1.2 mm paracentesis and uses high-frequency diathermy delivery to place six small sclerotomy pockets in the iridocorneal angle, which provide an improved outflow of the aqueous humour.

  • The wireless and dual linear foot pedal is the multifunctional control centre of the OS 4. Manufactured from robust metal, the pedal responds precisely and without delay to the surgeon’s commands.

    Dual Linear Versatility

    In the OS 4 pedal, the dual linear control can be perfectly matched to the surgeon’s wishes and needs. The modern multifunctional pedal allows comfortable and independent work while boasting intuitive navigation throughout all surgical steps. Six auxiliary buttons can be assigned to various functions, such as switching between functions or colour settings. Over 100 setting options are available.

    Integrated Laser Pedal

    Laser function under perfect control: When using the integrated endo laser, the pedal becomes a true laser pedal – immediately ready for use and controllable without delay.

  • In its OS 4 surgical platform, Oertli has united the very best from two worlds – the peristaltic pump and the Venturi pump. These two pump systems have become established in eye surgery. With the Peristaltic pump, the flow is created mechanically. While the vacuum can only be induced under occlusion and isn’t directly controlled, it boasts the advantage that the flow can be very finely tuned. This creates positive results, especially with maneuvers that require a high degree of delicacy. The Venturi pump, on the other hand, creates the vacuum through an air current. The vacuum is built-up quickly and is linearly adjustable, however, the flow is not directly controlled.

    Three pump options are at the surgeon’s disposal: Peristaltic, Venturi and SPEEP. Depending on the situation, the surgeon has the ability to choose the best suited pump. The preferred pump can also be stored for each programme. The SPEEP pump is based on the flow-based Peristaltic pump. Both the flow (up to 60 ml/min) and the vacuum (up to 650 mmHg) can be precisely controlled. The vacuum can also be adjusted, even with occlusion, ensuring safe and stress-free work with maximum efficiency.

  • The OS 4 boasts a highly developed surgical technology. This does not mean, however, that the operating platform is difficult to use. On the contrary: everything about the OS 4 is clear and easy. This makes operation comfortable and easy to understand for both OR personnel and the surgeon. The operating platform is ready for start extremely quickly thanks to its own software technology. Following a switch-on time of about 30 seconds, the system is ready for start. This makes changeover between surgeries very short.

    New Feature: Temporarily deactivate irrigation

    In retinal surgery, irrigation can be deactivated for a defined period (Irrigation Off), such as when the surgeon would like to lower the IOP to examine the peripheral retina.

    Touch glass screen

    The no-frills 15-inch touch screen with a high-quality glass front directly leads the eye to the right function. The graphic user interface is simple and surgeon and OR personnel are immediately in focus and master functions intuitive to operate.

    Voice confirmations

    User-friendly voice confirmation in five individually adjustable languages (English, German, French, Italian, Spanish) enable focused and autonomous work throughout the surgical procedure. The acoustic laser protection signals provide additional operating safety for the entire surgical team.

  • Brighter, more homogenous and safer

    Thanks to the Power LED light source, the light output is up to 45 per cent greater than the previous generations. The OS 4 has a double light source that provides homogeneous illumination and high durability thanks to its latest Power LED technology. The wide control range is an ideal combination with 3D microscopes, especially in case of low lumens.

    Power LED Plus

    The colour spectrum of the second light source, the Power LED Plus, can be individually adjusted. To make even the finest tissue structures visible and to enable high-resolution contrast vision, the colours yellow and blue can be mixed as needed.

  • The OS 4 contains a fully integrated green endo laser that has been developed and manufactured in Switzerland. The 532 nm laser can be used, among other things, to treat retinal problems such as retinal detachments, diabetic retinopathy and macular edemas retinal vein occlusions, peripheral retinal neovascularization central serous chorioretinopathy. Control is performed as you prefer, either by the 15-inch touchscreen or directly via the dual-linear pedal, which becomes a laser pedal following activation of the relevant function. The laser is ready for use in the shortest time possible, key switch, laser output and emergency stop lie beside other connections in ergonomically perfect arrangement. Displays and control are integrated in the graphic surface. This way, the endo laser becomes optimally integrated into the operational process.

    All Oertli laser probes are compatible with the endo laser, of course. Apart from the straight and curved laser probes, the steerable ones belong here as well. In this case, the surgeon controls the bending of the probe by pressing the hand piece – a characteristic enabling unimaginably precise control in the treatment of peripheral retina.

    Fully integrated and fully automated thanks to safety plus

    The fully automated laser user protection filter enables a reliable eye protection during treatment. The fully automatic insertion of the filter glass before the laser treatment saves the OR team precious time and energy. Since the filter glass is only active during laser treatment, the view always remains clear; there are no wavelengths of light that are filtered out. The acoustic laser protection signals provide additional operating safety for the entire surgical team.

    New feature: Personal laser settings accessible at all times

    Each surgeon can now save their personal laser values (Laser Power, Laser Duration and Pulse). Individual values can be programmed for up to 50 surgeons and can be readily accessed at any time.

  • Developed for a stable IOP

    An important criterion during eye surgery is a stable intraocular pressure (IOP). To maintain this IOP, the OS 4 offers two options of infusion systems: the active as well as the gravity infusion.

    Active infusion

    The gas-driven active infusion is available on the OS 4 and Faros devices. The OS 4 offers active infusion for procedures in the anterior and posterior segments. In the case of bleeding or other situations requiring high pressure, the active infusion can react quickly and reach a pressure of up to 120 mmHg. The active infusion is independent of the room height as the BSS bottle is suspended from a fixed hook. It is connected to the compressed air outlet (GFI = Gas Forced Infusion) via a Luer lock connection, whereby the connection is colour-coded to avoid installation errors.

    Gravity infusion

    The gravity infusion is mechanical driven and available on all three Oertli platforms: OS 4, Faros and CataRhex 3. Like the active infusion, the gravity infusion is also designed for stable IOP during surgery. Pressure can go up to 100 mmHg or 135 cm, and both setting values are selectable. As the tubing set is part of most phaco and vitrectomy single-use packs from Oertli, the gravity infusion is an economical solution.

    New feature: Quickly set infusion values

    The settings for the infusion values can now be changed faster with the foot pedal using five-unit increments (mmHg or cm).
